New Patients
Families can schedule ‘get-acquainted’ visits; however, these are not required to become part of PAFP’s practice.
Whether your first visit to our practice is for a newborn, toddler or adolescent care, please take the time to fill out our new patient forms located in our forms/policies section. We ask that you arrive 15 minutes before your scheduled appointment and bring your current insurance card with you. You can also print and complete our New Patient Health Questionnaire here. This will help expedite your insurance verification and make your office visit more efficient.
About Appointments
We encourage all visits to our office to be by appointment only. Illness visits are scheduled as needed, even the same day. Well child checks should be scheduled 4-6 weeks in advance. Extended consultation times are available for more complicated problems. Certain days of the week and times of the day are ‘peak’ times when the office will nearly always be busy. Well Visits
Our well visit schedule and vaccination schedule are in accordance with the American Academy of Pediatrics guidelines. You can find our general well child visit schedule by clicking here. Additionally, the physician may also recommend any catch-up vaccinations as needed. Athletic, camp and other well child exams are scheduled as needed.
